I, (parent/guardian), In consideration of Azul Soccer Academy allowing my child to participate in its programs, I HEREBY WAIVE, RELEASE, INDEMNIFY, HOLD HARMLESS, AND FOREVER DISCHARGE AZUL SOCCER ACADEMY, ITS AGENTS, EMPLOYEES, OFFICERS, DIRECTORS, AFFILIATES, SUCCESSORS, AND ASSIGNS, AND ANY OTHER PARTNER OR PROVIDER OF FACILITIES (HEREBY JOINTLY AND SEVERALLY REFERRED TO HEREIN AS "RELEASED PARTIES"), FROM ANY AND ALL CLAIMS, DEMANDS, DEBTS, CONTRACTS, EXPENSES, CAUSES OF ACTION, LAWSUITS, DAMAGES, AND LIABILITIES OF EVERY KIND AND NATURE, WHETHER KNOWN OR UNKNOWN, IN LAW OR EQUITY, THAT I OR MY CHILD EVER HAD OR MAY HAVE ARISING FROM OR IN ANY WAY RELATED TO MY CHILD’S PARTICIPATION.

This waiver and release include, but are not limited to, claims arising from any injuries to my child resulting from participation in any of the training, camps, or related soccer or physical activities conducted by, or on the premises of, or for the benefit of any of the Released Parties. This also includes injuries resulting from engaging in fitness or strength and endurance workouts, practicing and/or engaging in soccer practices, scrimmages, tournaments, league games, or other related activities, including travel to and from. I understand that these activities can be dangerous and involve risks of serious bodily injury, including permanent disability, paralysis, and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "Released Parties" named above; and that there may be other risks either not known to me or not readily foreseeable at this time. I fully accept and assume all such risks and all responsibility for losses, costs, and damages I and/or my minor child may incur as a result of my and/or my minor child’s participation.

On behalf of myself, my child, my heirs, assigns, next of kin, my spouse (if applicable), and I waive all claims for damages, injuries, and death sustained that we may have against the Released Parties regarding any such activity. I certify to the best of my knowledge that my child’s current physical condition is satisfactory for participation in Azul Soccer Academy’s programs and that my child is free of any health problems that would endanger his/her participation. I will inform the coach should my child’s condition change at any time during participation in this program. This waiver of liability does not apply to any acts of gross negligence or intentional, willful, or wanton misconduct.

I further agree that in consideration of and as a material term of Azul Soccer Academy’s agreement to allow my child to participate, I AGREE TO INDEMNIFY, DEFEND, AND HOLD AZUL SOCCER ACADEMY HARMLESS FOR THE COST OF DEFENDING AND SATISFYING ANY CLAIM THAT IS FILED AGAINST AZUL SOCCER ACADEMY BASED ON OR ARISING FROM INJURIES TO MY CHILD.

This waiver and release contain the entire agreement between the parties and supersede any prior written or oral agreements concerning this subject matter. The provisions of this waiver may be waived, altered, amended, or repealed, in whole or in part, only upon the prior written consent of all parties.

I have read, understand, and fully agree to the terms of this waiver and release. I understand and confirm that by accepting this waiver and releasing said child, I have given up considerable future legal rights. I have accepted this Agreement freely, voluntarily, and under no duress or threat of duress, without inducement, promise, or guarantee being communicated to me. By completing this registration, I am executing a complete and unconditional waiver and release of all liability to the full extent of the law.

I, hereby grant and assign to Azul Soccer Academy Austin ("Azul Soccer Academy") and its legal representatives the right and permission to publish my child’s name, likeness, photographic image, video, and biographical data for the specific purpose of publicity and promotion of the player, team, and/or Azul Soccer Academy. You may request the removal of any media at any time.

I expressly agree that the foregoing Release Waiver is intended to be as broad and inclusive as is permitted by the law of the State of Texas and that if any portion thereof is held invalid, it is agreed that the balance shall continue in full legal force and effect.
