I, the undersigned parent or legal guardian, in consideration of Azul Soccer Club permitting my child to participate in its programs, hereby WAIVE, RELEASE, INDEMNIFY, HOLD HARMLESS, and FOREVER DISCHARGE Azul Soccer Club, its agents, employees, officers, directors, affiliates, successors, assigns, and any partner or provider of facilities (collectively referred to as the “Released Parties”) from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages, and liabilities of any kind—known or unknown, in law or in equity—arising from or in any way related to my child’s participation in Azul Soccer Club programs.

This waiver includes, but is not limited to, claims arising from any injuries sustained during training sessions, camps, league games, tournaments, travel, or other physical or soccer-related activities conducted by or for the benefit of the Released Parties. I acknowledge that these activities carry inherent risks, including serious bodily injury, permanent disability, paralysis, and death, which may be caused by my child’s actions or inactions, the actions or inactions of others, facility conditions, or negligence of the Released Parties. I voluntarily assume all such risks and accept full responsibility for any losses, costs, or damages resulting from my child’s participation.

On behalf of myself, my child, our heirs, assigns, next of kin, and spouse (if applicable), I waive all claims for damages, injuries, or death sustained in connection with participation in any Azul Soccer Club activity. I certify that my child is in good physical health and has no condition that would prevent safe participation. I will notify coaches if my child’s condition changes at any point.

This waiver does not apply to acts of gross negligence or intentional misconduct.

Furthermore, in consideration of Azul Soccer Club allowing my child’s participation, I agree to INDEMNIFY, DEFEND, AND HOLD HARMLESS Azul Soccer Club from any costs or claims arising from injury to my child.

This document constitutes the entire agreement and supersedes any prior understanding related to this matter. Changes to this waiver must be made in writing and agreed to by all parties.

I have read and voluntarily accept this waiver, fully understanding its legal consequences. I confirm that I am not under duress and am not relying on any promises or guarantees not included herein.

I agree that this waiver is intended to be as broad as permitted under Texas law, and if any portion is found invalid, the remainder will continue in effect.
